- Position Type: Full-time, 40 hours/week
- Schedule: Monday–Friday, no evenings or weekends
- Work Environment: Fully remote
- Requirements:
- 2–5 years of Chronic Care Management experience
- Own computer with webcam (tablets/iPads not supported)
- Reliable home Wi-Fi connection
- Previous remote work experience preferred
- Strong multitasking and problem-solving abilities
- Strong organizational skills and attention to detail are required to manage multiple tasks efficiently
- Excellent communication skills, both verbal and written, to interact effectively with patients and team members
- Compensation & Benefits:
- $21-$24/hr

- About the Role:
- As the Chronic Care Coordinator, you would be responsible for coordinating and continually monitoring the care of patients with chronic conditions in our chronic care management (CCM) program, collaborating closely with physicians, nurses, social workers, pharmacists, and community partners to help ensure they meet their health goals.
- In this role, you would develop and update personalized care plans, conduct routine follow-ups, educate and empower patients on self-management, and connect them with essential community and financial resources. Success in this role requires your willingness to go above and beyond for each patient, a scrappy resourcefulness to uncover creative work-arounds when traditional pathways stall, and strong complex problem-solving skills to untangle multifaceted clinical and logistical challenges. In addition, an ability to think outside the box to design innovative care strategies that improve engagement, adherence, and outcomes is needed in this role. This position also requires a strong understanding of medical terminology and experience in a medical office setting.
- Responsibilities:
- Conduct comprehensive patient assessments to develop individualized care plans
- Collaborate with interdisciplinary care teams, including hospitals and primary care providers
- Monitor patient health status and facilitate follow-up care
- Educate patients and families about self-management, medications, and available resources
- Maintain accurate and timely documentation in compliance with regulatory standards
- Advocate for patient needs and coordinate services to ensure continuity of care

Pay: $21.00 - $24.00 per hour
