Job Description
An Embrace Prevention Care Clinician (PCC) is responsible for helping older adults with two or more chronic conditions learn to take preventive actions that improve their overall healthspan. Our goal is to reduce their chance of a hospital admission and extend their ability to independently age in place. You will be responsible for their health outcomes by changing the behaviors of a patient panel of approximately 50 patients for every 10 hours per week you work. You will build long-term trusting relationships with each patient through frequent telehealth interactions.
We are currently focused on GUIDE which is a new CMS Innovation Center 8-year pilot that started July ’24. The goal of GUIDE is to provide preventive, longitudinal care to Medicare beneficiaries with dementia as well as support services to their caregivers to reduce hospitalizations and the need for skilled nursing.
The GUIDE Dementia Care provided by Embrace Nurse Practitioners as Care Navigators includes:
- Care Navigators help patients and their caregivers to establish and then follow their personalized dementia care plan.
- Care team coordination to improve patient care and reduce caregiver burden.
- Caregiver education and support
- Coordination of temporary respite services for caregivers with Medicare paying up to $2,500 per year
- No cost or copay for qualifying Medicare beneficiaries
Embrace’s nurse practitioners serve patients in their local area. You will establish relationships with other local healthcare providers serving the older adult population and become known as an outstanding care provider and advocate for senior health. You will work closely with the patient’s care giver, primary care physician and neurologist (if applicable) and provide updates on the patient’s progress and make recommendations for their care to the physician and practice. You will also coordinate and communicate with the patient’s other medical providers, pharmacy, other healthcare providers and family caregivers.
You will mostly work from home and manage your schedule to meet the needs of your patients and the company. Sometimes, you may need to meet with the patient at the provider's office or in the patient’s home. Travel expenses will be reimbursed.

- Position Requirements:
- Nurse practitioner (APRN)
- Must be Fluent in Spanish
- Living in and actively licensed in Ohio
- Minimum of 5 years of experience in direct patient care, preferably in family medicine, chronic care, or geriatric care
- Ability to work primarily from home with some local travel
- Ability to work flexible days and times to meet individual patient needs
- Proven experience and competency using computer systems, Microsoft/Google Office products, Electronic Health Records, State Health Information Exchanges, etc.
- Ability to build trusting relationships with patients, physicians, and colleagues
- Willingness to be an advocate for your patients and take the necessary actions to keep them healthy
- Preferred Skills
- Clinical knowledge and treatment of patients with Dementia
- Clinical knowledge to handle complex patients and make recommendations to physicians
- Provide exceptional care and empathy for your patients
- Proven track record of achieving patient health outcomes
- Ability to treat each patient individually and help them do things they couldn’t have done on their own
- Demonstrate a creative ability to identify and articulate ways to improve our processes to improve patient health outcomes
- Ability to work independently and remotely
- Ability to collaborate with other team members remotely to discuss complex cases, reasons for hospitalizations and/or Emergency department visits, learn from and share best practices with colleagues
- Confidence and optimism to take risks to improve patient outcomes

Compensation
A competitive hourly salary will be provided

Pay: $60.00 - $65.00 per hour
