Role Description
- The Sr Clinical Care Navigator is responsible for providing care navigation support to assigned accounts utilizing products of Exact Sciences. This role actively engages with health systems and patients to provide the highest level of customer service to ensure positive patient outcomes.
- Develop relationships with health care systems, vendors, and patients
- Understand and identify various community resources
- Develop and maintain care navigation workflows
- Answer queries regarding care navigation as primary responsibility
- Liaise with cross-functional internal and external teams to maintain a high level of customer service
- Essential Duties Include, but are not limited to:
- Assist in implementing, co-creating, and updating care navigation workflows for health systems
- Identify and execute effective solutions to address inquiries
- Provide care navigation insights, assist in identifying community resources, and operational support to health care systems
- Advocate health system needs around care navigation with all Exact Sciences product teams
- Work through customer or patient concerns with Customer Support Teams
- Partner with internal teams to develop efficiencies and process improvements
- Incorporate customer and manager input into service delivery
- Build effective relationships
- Analyze diverse problems and select appropriate methods for solutions
- Handle multiple projects while remaining organized
- Uphold company mission and values through accountability, innovation, integrity, quality, and teamwork
- Support and comply with the company’s Quality Management System policies and procedures
- Attend work regularly and reliably
- Ability to work nights and/or weekends, as needed
- Ability to work standing or seated for approximately 50% of a typical working day
- Ability to work on a mobile device, tablet, or in front of a computer screen and/or perform typing for approximately 90% of a typical working day
- Ability to work on a computer and phone simultaneously
- Ability to use a telephone through a headset
- Comply with any applicable personal protective equipment requirements
- Qualifications
- Bachelor’s degree in Population Health or related field; or high school degree/general education diploma and 4 years of relevant experience in lieu of Bachelor’s degree
- 2+ years of clinical experience working in health care system
- Proficient computer skills to include Internet navigation, Email usage, and word processing
- Proficient in Microsoft Office to include PowerPoint and Excel
- Demonstrated ability to perform the Essential Duties of the position with or without accommodation
- Authorization to work in the United States without sponsorship
- Requirements
- Experience in Quality, Population Health, Community Health, or Care Navigation within a health system (preferred)
- Clinical degree or certification, i.e., RN, BSN, LPN, RMA, CMA, etc. (preferred)
- Proficient in the use of Microsoft Visio (preferred)
