PCN Clinical Pharmacist
Boston Primary Care Network is recruiting Pharmacists who are passionate about using their extensive skills and expertise to maximise the benefits patients receive from medications. You will join and further develop their forward-thinking Clinical Pharmacy and Medicines Optimisation team. The team is well established withinthe PCN and includes experienced Clinical Pharmacists alongside a Pharmacy Technician. Applications from newly qualified Pharmacists will be considered.

The ideal candidate(s) will relish the opportunity to work independently as required but also collaboratively with various members of the MDT across the PCN including butnot limited to GPs, nurses, pharmacy colleagues, physiotherapists,occupational therapists and paramedics. They will use an individualised approach to proactively achieve medicines optimisation for patients within the Boston Primary Care Network.
Applications are welcome from pharmacists with patient-facing experience from any sector, as well as newly qualified Pharmacists. Whilst relevant previous experience in general practice is desirable, this is not essential; the structured training pathway provided by CPPE coupled to the support provided within the PCN by Clinical Pharmacist colleagues will enable candidates committed to continual development to be successful in the role.
For candidates that are already independent prescribers, you will be supported with further development and education via the Advanced Clinical Practice pathway established by Health Education England as appropriate.

We are a group of 6 general practices within the Boston area who proactively champion multi-disciplinary team working, and recognise Pharmacists are an essential component as expertsin prescribing and medicines optimisation.
This is an exciting opportunity for Clinical Pharmacists to develop their own role combining remote working alongside contact time within surgeries, which will be managed flexibly and according to service need. As you develop your clinical assessment skills therewill be an expectation to conduct face to face patient reviews to optimise patient care accordingly.
Job responsibilities
The post holder will relish the opportunity to work independently as required but also collaboratively with various members of the MDT across the PCN including GPs, nurses, clinical pharmacy colleagues, physiotherapists, occupational therapists and paramedics,using an individualised approach to proactively achieve medicines optimisation for patients withinthe Boston Primary Care Network. Additionally, the post holder will be required to support the COVID-19vaccination programme according to service need.
The post holder will work within their clinical competencies as partof a multi-disciplinary team to provide expertise in clinical medicines management. This will involve; providing structured medication reviews,managing long term conditions, management of medicines on transfer of care and systems for safer prescribing, managing repeat prescription authorisations and reauthorisation, acute prescription requests, and addressing both the public health and social care needsof patients in the GP practice(s) that make up the PCN.
The post holder will performa patient-facing role including structured medication review of patients with polypharmacy especially for older people, people in residential care homesand those with multiple co-morbidities.
The post holder will provide leadership on quality improvement and clinical audit. This role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver an excellent service within general practice.
- The post holder will be supported to develop their role to become a non-medical prescriber, if that qualification is not already held. It is anticipated that responsibilities ofthe role will develop in accordance with experience and service requirements.Person Specification Experience
- -Experience and awareness of the breadth of common acute and long term conditions that are likely to be encountered in General Practice
- -Experience of partnership/collaborative working and building relationships across organisations
- -Demonstrates ability to integrate General Practice with community and hospital pharmacy teams and other care providers across the PCN Qualifications
- -Completion of an undergraduate degree in Pharmacy
- -Registration as a qualified Pharmacist with the General Pharmaceutical Council (GPhC)
- - Postgraduate Diploma in Clinical Pharmacy
- - Completion of the CPPE Primary Care Pharmacy Education Pathway
- - Registered as an Independent Prescriber with the GPhC Disclosure and Barring Service Check
