The RN will provide patients who have chronic conditions the support, education, and assistance in the prevention and/or maintenance of their disease and/or health and wellness state. Will work with the individual patient, their families, and providers of care to develop and implement individual health plans and provide support to the patient in self-management. Coordinate care and/or follow-up for patients who receive care in outpatient facilities—coordination of prescription refill requests and patient education coordination in collaboration with physician staff. Commitment to patient outcomes results in high levels of performance despite workload changes. This position functions in the role of charge nurse. Qualifications: Minimum Qualifications: Graduate of an accredited Nursing Program with an Associate's Degree, Nursing Diploma, Bachelor's in Nursing, or Master's in Nursing ; Valid RN License; Minimum of 1 year Registered Nursing experience. Preferred Qualifications: Current BLS certification. Supervisory experience. BSN Preferred Additional Information: Responsibilities: Delivers nursing care according to the standard of care defined by UAMS, ANA, and Standards for specialty patient population with support and mentoring by other team members. Collects and documents assessment data; Develops a Plan of Care (POC) per standard. Implements POC, including medical order and interdisciplinary approach. Evaluates the progress of the patient toward the attainment of desired outcomes. Provides effective patient/family education/discharge planning. Administers medication per standard, including drug information to patients/families. Acts as a resource/supports others. Delivers patient care using the principles of the defined nursing modality. Delegates care based on UAMS standards, legal and regulatory requirements, specific job description, and individual level of education and training. Contributes to a positive work environment that is conducive to clinical education while serving as a resource for student nurses and staff. May perform other duties as assigned.
